Case Management

Comprehensive Case Management meets the needs and goals of our veterans and their families through communication, available resources and support.  The case manager meets with the veterans and/or family members to assess their needs, build on their strengths, and support them in achieving their goals.  Our case manager works with participating agencies, whenever necessary, to ensure that our veterans and their families’ needs are being met.  The case manager follows up with veterans or family members throughout the process and after services have been provided.  We are committed to your success; we are here every step of the way.
