Forensic A.C.T (FACT) (Commencing in July 2025)

Forensic Assertive Community Treatment (FACT) supports adults who live with severe mental illness who are involved with the criminal justice system. These individuals often face complex, intersecting challenges, including severe psychiatric symptoms, substance use disorders, and limited access to social services and are often at an increased risk of reoffending and may struggle to access traditional mental health services due to factors such as mistrust, lack of insight into their condition or barriers like transportation and financial instability. FACT receives referrals from SPOA, is comprised of prescribers/medical staff, social workers/clinicians, peers/credible messengers and criminal justice specialists, and will provide in-home services to clients several times a week.

Who is eligible?

Adults aged 18 and older, who are justice impacted and living with serious mental illnesses and have not been successfully engaged by the traditional mental health treatment and rehabilitation system. Individuals must have current involvement with the criminal justice system.
